Generally, the correct on set date is based on medical facts. That means if you were involved in an accident, the date of the accident might be the date of the on set if you were disabled and unable to work as of that date. If a) disability onset established by SSA as of the date that is no later than 60 days after the Cobra qualifying event (the end of your employment); and b) the Social Security determination is presented to the plan at the end of the original 18 month Cobra.

This Cobra extension can be for an additional 11 months and changing the on set date for a period beyond the end of your original 18 month Cobra period can result in you losing your Cobra extension.
