Social Security District Offices

The two Social Security Disability programs that you may be eligible for are Disability Insurance Benefits (DIB/SSDI) and/or Supplemental Security Income (SSI). You may file your application online, in person at your local Social Security District Office (DO), or over the telephone (1-800-772-1213). It can take several hours to file an application. In addition to filing the application form for DIB and/or SSI, the claimant must complete the Adult Disability Report (SSA-Form 3368).

The Social Security Administration (SSA) makes a technical determination as to which disability programs you may be eligible for and gathers the necessary documentation to complete your application. After the application is complete SSA transfers the file to the Disability Determination Services.

It is important to hire an experienced attorney who understands the disability claims process. Many times the SSA dismisses claims as, "abandoned" if all of the documentation is not completed properly and in a timely manner. Our law firm works with you to complete all of your applications, appeals, and Disability Determination questionnaires.
